What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Roselyn Villaver, and explained the reason for the visit. The licensee/administrator, Myline Olivas was called and came to the facility shortly after. At approximately 10:00am, with the assistance of staff, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are hardwired and interconnected. There are carbon monoxide detectors that functions properly. The fire extinguisher is located in the kitchen. The charge date is 08/09/24. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer in the kitchen. Properly labeled medications were locked in one of the kitchen cabinets. A complete 1st aid kit is maintained in the kitchen. Bedrooms: There are six (6) bedrooms. Five (5) bedrooms are designated for the residents' use. One (1) bedroom is designated for staff. Of the five bedrooms designated for resident use, four (4) are private and one (1) is shared. LPA observed that all resident rooms were properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 109.8 to 112.7 degrees Fahrenheit. No cleaning supplies observed in the bathrooms during the day's visit. Common Areas : These included the living room and dining area. The common areas were properly furnished. The auditory alarms on main exit doors were on and functional at the time of the visit. OFFICE/STAFF WORKSTATION: Staff office is located by the living room. Resident and personnel files are maintained in a locked filing cabinet there. Surrounding Grounds : Entry/exits were free of obstruction. There was sufficient yard space in the back yard for outdoor activities. The laundry area and detergents are located at the backyard. Detergents and cleaning supplies were observed stored and locked during the day's visit. There is also a shed in the backyard that is used to store supplies and disposable items. Shed was locked during the day of the visit. Back gate leads to an alley. Back gate is utilized by paramedics and first responders for resident transport. Resident Files : LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files : LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications : Medication and Medication Records were review for proper storage, documentation and implementation. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Exit Interview Conducted and a Copy of the Report Issued.
