What the inspector wrote

Licensing Program Analyst (LPA) Brian Balisi conducted a collateral visit to this facility in regards to a to complaint at another location (Complaint control # 31-AS-20200117130958). Upon arrival LPA met with Administrator Matthew La Vine and explained the reason for the visit. The purpose of the visit was to conduct an interview with Staff #1 (S1) . A brief interview was also conducted with administrator regarding S1 Exit interview conducted and copy of report issued.
