Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Tuesday Cabiness arrived at the facility to conduct an unannounced annual visit. Upon arriving, LPA was greeted by caregiver Jemma Barseghyan, who allowed LPA to enter the facility; she was informed the reason of the visit. Staff contacted Administrator Arsen Grigoryan who came shortly after. The current census is (6). LPA observed several postings upon entry, including an Administrator’s Certificate, Resident Rights, Complaint Poster, Emergency and Disaster Plan, and Evacuation Procedures. The physical plant inspection was conducted. The facility has (5) bedrooms; with (4) private rooms, and (1) shared. Kitchen: LPA noted adequate food preparation space in the kitchen. All equipment appeared in good repair. LPA observed sufficient perishable and non-perishable food, as well as surplus of beyond 7 days. All sharp objects, toxins, and hazardous items were locked. Bedrooms: LPA observed a chair, night stand, a lamp, and a chest of drawers. LPA also observed sufficient clean blankets, bedspreads, top bed sheets, bottom bed sheets, pillow cases, and mattress pads. Bathrooms: LPA observed grab bars inside the showers and beside the toilets, along with non-skid mats and shower chair. All bathrooms also had fully stocked soap, paper towels, toilet paper, and trash cans had lids. LPA measured the the water temperature at 113.4 degrees Fahrenheit. Laundry : LPA observed washer and dryer outside and in good repair. Detergents and hazardous chemicals were locked up. Outside: Walkways, ramps, and handrails were clean, secure, and free from obstruction. LPA observed an locked shed. Patio furniture observed for resident's comfort. Exit gates were unlocked and accessible. No bodies of water or swimming pool on the property. LPA observed a locked medication cabinet. First aid kit fully supplied and fire extinguishers were fully charged. Smoke detector and carbon monoxide detectors were operating. Alarms on doors were working. Facility Records: The facility files are kept in cabinet in the dining area. Staff and resident files all had Licensing required documents. No citation issued, exit interview conducted and copy of report provided to Administrator.
