What the inspector wrote

Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted an unannounced collateral visit in reference to complaint control #31-AS-20241025111603, regarding another licensed facility. Upon arrival LPA met with staff Satenic Kilajian. LPA explained the purpose of the visit. An entrance interview was conducted. At 10:30 AM LPA conducted a physical plant tour of the facility with the assistance of staff. At 10:45 AM. LPA Ngo-Castaneda reviewed and obtained copies of records relevant to the investigation. At 10:55 AM Licensee arrived Rima Agaronyan and explained the reason of the visit. LPA conducted an interview with R1 and S1. Copy of LIC 809 provided, exit interview conducted.
