Inspector’s narrative

What the inspector wrote

On 5/09/2025, Licensing Program Analyst (LPA) Melissa Spaeth conducted a complaint investigation for Complaint #31-AS-20250508155156. During LPA's visit, LPA observed a staff member who has not been fingerprint cleared. The facility manager confirmed the staff member had been working at the facility for the four past days. The LPA Spaeth spoke to the Licensee, Jerome Viray at 12:45 pm. LPA Spaeth stated to the Administrator the staff member must immediately leave the facility. LPA Spaeth the staff member left the facility 1:00 pm. LPA toured the facility at 1:00 pm until 1:15 pm. LPA observed the delayed egress device to the sliding glass door was not working. LPA observed the facility manager turned on egress device at 1:30 pm. Due to LPA observations, the following deficiencies were issued (see 809-D). Exit interview, appeal rights discussed, and a copy of the report was given.

This is evidenced by: Based on LPA's observations, the licensee did not comply with the section cited above. LPA observed the delayed egress alarm to the sliding glass door had been turned off. which poses an immediate health, safety or personal rights risk to persons in care.
