What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Armine Harutyunyan, and and advised her of the visit. At approximately 9:30am, LPA took a tour of the physical plant. The facility is a one story building. Required postings were observed in the entry and living room area. The smoke alarms and carbon monoxide are dual and interconnected. There are three fire extinguishers. One is located in the kitchen, and the other two is located in the garage. The charge date for all three fire extinguishers is 10/09/25. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives were stored in a locked drawer. Bedrooms: There are four (4) bedrooms designated for residents' use. Bedrooms #1 and #3 are private. Bedrooms #2 and #4 are shared. All bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 111 and 113 degrees Fahrenheit. No cleaning supplies were observed in either bathrooms during the day of the inspection. Common Areas : These included the living room and dining area. The living room was furnished with sufficient seating, two couches, recliner, table and television. The fireplace was properly screened. No tools present. The dining room table is large enough to seat up to six (6) individuals. Both the living room and dining room furniture were in good repair. Floors were mopped and clean. Exits and passageways were clear. Surrounding Grounds : Backyard has a patio area with furniture appropriate for outdoor use. There is a storage building in the backyard for excess furniture. Backyard is large to hold outdoor activities. Gates at the side of the home presents no exterior lock and is clear to exit/evacuate in case of an emergency. Laundry area : The laundry area is located in the garage. Toxins and detergents were stored and locked. Staff Workstation/Office : There is a staff workstation located at the corner of the kitchen where resident and staff files are maintained Resident Files : LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files : LPA conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications : Medications are stored in a locked cabinet in the kitchen. Medication and Medication Records were reviewed for proper storage and documentation. There is a complete first aid kit and manual stored in medication cabinet. Garage: The garage is attached to the building. In addition to the laundry area, the garage is used for storage for cleaning supplies, emergency water and PPE supplies. Entry to garage is through the kitchen, past the staff work station. Garage is kept locked and inaccessible to the residents in care. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the day's visit. Administrator was advised and a copy of this report issued.
