Licensing Program Analyst (LPA) Leslie Ngo-Castaneda met with facility licensee Adrine Akopyan for a Plan Of Correction (POC) visit. The purpose of the POC visit is to make sure deficiencies were corrected on reports issued on 2.12.2025 Entrance interview conducted. At 9:35 AM LPA toured the home and requested the following: -CCR 87465(a)(4) Incidental Medical and Dental Care Services: POC: Based on record review, the licensee did not comply with the section cited above in 1 out of 1 medication bubble pack were randomly punch, that results in inconsistency, which poses/posed a potential health, safety or personal rights risk to persons in care. POC date 2.19.2025: Licensee agreed to do a vendor training for medication dispensing. - CCR 87457(c)(1)(a) Incidental Medical and Dental Care Services: POC: Based on record review, the licensee did not comply with the section cited above in 1 out of 1 R3 LIC 301, appraisal and pre-placement is missing, which poses/posed a potential health, safety or personal rights risk to persons in care. POC date 2.24.2025: Licensee agreed to have R3 pre-placement filled and signed. Exit interview conducted, copy of this report signed and delivered.
