What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Genya Sukiasyan, and explained the reason for the visit. The administrator, Nelli Berberyan, was notified over the telephone. At approximately 9:45am, LPA took a tour of the physical plant. Required postings were observed in the living room and entry area. The smoke alarms and carbon monoxide detector are dual, hard wired and inter-connected. The facility has one new fire extinguisher, located by the kitchen. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer. Cleaning supplies were stored locked underneath the kitchen sink. Bedrooms: There are four (4) bedrooms designated for client use. Bedroom #1 and #4 are shared rooms, Bedrooms #2 and #3 are private rooms. Bedrooms designated for resident use is properly furnished with beds, night stand, chairs, dresser, bedding and linen. All resident bedrooms have sufficient lighting and closet space. Bathrooms: There are three (3) bathrooms designated for residents' use. Bedroom #4 has it's own bathroom. All bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 105 and 108 degrees Fahrenheit. No cleaning supplies were observed in any of the bathrooms during the day of the inspection. Common Areas : These included the living room, activity room, and dining room. The living room is observed with two couches, recliner, center table, and a television. The dining room table is large enough to seat six (6). There is an activity room located adjacent to the dining area. Furniture were in good repair, and the floors were mopped and clean. Surrounding Grounds : Passageways and entrance were clear of obstruction. All entry and exit doors have a functional auditory alert. The left side of the home and backyard of the facility has no access to the residents. There is an ADU on the left side of the facility, that has it's own private entrance. No accessibility to the residents. The exterior, right side of the facility is for resident use only. This side is furnished with outdoor furniture and has sufficient yard space. Laundry area : The laundry room is located by the garage. No toxins and detergents present. Staff Workstation/Office : Staff workstation is located at the corner of the activity room. Resident and personnel files maintained in a locked filing cabinet there. Resident Files : LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files : LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications : The medication cabinet is located in the kitchen. Medication and Medication Records were reviewed for proper storage and documentation. Garage: The garage is attached to the building, located adjacent to the laundry room. It is used as extra storage space. No resident access there as there is a locked door to gain entry through the laundry room, to get to the garage. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Exit Interview Conducted, and a copy of this report issued.
