What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Hachat Kachataryan, and explained the reason for the visit. The administrator, Gohar Ambartsumyan, was advised, and came to facility shortly after. At approximately 9:45am, with the assistance of staff, LPA took a tour of the physical plant. The facility is a one story building. Required postings were observed in the entry area. The smoke alarms and carbon monoxide are dual and interconnected. There is one fire extinguisher that was purchased on 10/15/25. Fire Extinguisher is located in the kitchen. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer in the kitchen. Cleaning supplies were stored locked underneath the kitchen sink. Bedrooms: There four (4) bedrooms designated for residents' use. Bedrooms #1 and #2 are private. Bedroom #3 and #4 are shared. All bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 105 and 107 degrees Fahrenheit. No cleaning supplies were observed in either bathrooms during the day of the inspection. Common Areas : Common areas include the living room and dining room. The living room is furnished with sufficient seating, couches, recliners, tables and television. The dining room table is large enough to seat up to six (6) individuals. Furniture in common areas are observed to be in good repair. Floors were mopped and clean. There is no fireplace. Entry/exits, hallways and passageways were clear. Surrounding Grounds : Entry/exits to the front and back yards were free of obstruction. There is an enclosed patio area at the front where activities can be held. There is a storage building also at the front for supplies. It is kept locked. The pool is located in the backyard fenced, locked, and inaccessible to residents. Laundry area : Laundry is located within the hallway. Dryer and washer observed to be in good repair. No Detergents or fabric softener present during the inspection. Staff Workstation/Office : There is a staff workstation located at the corner of the kitchen. Resident Files : Resident files are kept locked in the medication cabinet. LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files : Staff files are also kept locked in the medication cabinet. LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications : Medication cabinet is in the kitchen. It is kept locked at all times. Medication and medication records were observed for proper storage and documentation. Garage: The garage is separate from the facility, It is currently being used for storage. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Exit Interview Conducted and a Copy of this Report Issued.
