What the inspector wrote

On 9/25/2025, Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit and was greeted by the Administrator, Rey Medel. LPA stated the purpose of the visit was to conduct an annual inspection. The Administrator confirmed there are two residents living in the facility. The facility is licensed for four non-ambulatory residents. The Administrator and LPA Spaeth toured the facility from 11:00 am until 11:20 am. LPA observed the following: Common Area – LPA observed the living room, dining room, and kitchen are combined. The living room area contains comfortable seating. The dining room area contained a dining room table, chairs, and a locked cabinet to store resident and staff files. The kitchen area contained a seven day supply of non-perishable food and a two day supply of perishable foods. A locked kitchen cabinet contained the residents’ medication and the first aid kit. The cabinet underneath the kitchen sink contained cleaning solutions and laundry detergent. There is a locked drawer for knives. A fire extinguisher is also located in the kitchen. A pantry is located in the kitchen and the washer/dryer is stored in this area. Backyard - The back yard contained comfortable seating. Continued on 809-C Bathrooms - There is a half bath and two full bathrooms which contained hand soap, paper towels, trash can, grab bars, and slip resistant mats. Water Temperature - LPA tested the water temperature at 11:10 am and the temperature was 110.0 degrees F. Smoke/Carbon Monoxide Detectors – the detectors were tested at 11:13 am and were operable. Resident Files – LPA reviewed resident files from 11:20 am until 11:40 am. LPA Spaeth also checked the resident's medication. Staff Files - LPA reviewed staff files from 11:40 am until 12:10 am. There are no deficiencies to report at this time. Exit interview conducted and a copy of the report was given.
