What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Lilit Stepanyan, and explained the reason for the visit. At approximately 10:45am, with the assistance of the administrator, LPA took a tour of the physical plant. The facility is a one story building, licensed to serve residents age 60 and over. Required postings were observed by the entry area. The smoke alarms and carbon monoxide are dual, hardwired and interconnected. There is one fire extinguisher, located in the kitchen. The charge date is October 24, 2024. The last emergency fire/evacuation drill was conducted on August 1, 2025. Kitchen: The kitchen appliances and fixtures were functional. LPA observed a sufficient amount of perishable and non-perishable food. Knives were stored in a locked drawer in a cabinet. Cleaning supplies were stored locked in another cabinet underneath the kitchen sink. BEDROOMS: There are four (4) bedrooms designated for client use. Bedroom #1 and #2 are private. Bedroom #3 and #4 are shared. Bedroom #2 has a fire clearance for bedridden (per STD 850). All bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting. BATHROOMS: The facility has three (3) bathrooms. Bedrooms #3 and #4 has it's own bathroom with shower, sink and toilet. There is a common bathroom located in the hallway, by bedroom #1. All three bathrooms were observed to have the proper fixtures, grab bars, and non-skid floors. The hot water delivered in the bathrooms measured between 105 to 110 degrees. COMMON AREAS: These included the living room and dining area. The living room is equipped with coaches and sufficient seating with space in between. The dining room table is large enough to accommodate six (6) residents. Floors were mopped and clean. Furniture is in good repair. Hallways and passageways were clear of any obstruction. There is no fireplace. All exit doors were checked to insure that the auditory alert is functional. LAUNDRY AREA: The laundry area is located by Room #3. The washer/dryer is maintained and in good repair. No detergents and softener observed during inspection. MEDICATIONS: The medications and medication records are kept locked in an upper cabinet, in the kitchen. Medications and medication records were reviewed for proper storage and documentation. SURROUNDING GROUNDS: The front and backyard is gated. There are no exterior locks making passageways, entrance and exits clear of any obstruction. There are no bodies of water. There is a small patio area with outdoor furniture to accommodate the six (6) residents. There is a storage cabinet where supplies are kept locked. Garage: No garage. Resident Files : LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files : LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Exit Interview Conducted and a Copy of this Report Issued.
