Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Melissa Spaeth arrived at 10:30 am and conducted an unannounced visit and was greeted by two caregivers. The Administrator Renelyn Landicho arrived at 11:30 am. LPA stated the purpose of the visit was to conduct an annual inspection. The staff members confirmed there are two (2) residents. The facility is licensed for six (6) non-ambulatory residents of which one (1) may be bedridden in room 4. LPA toured the facility from 11:10 am until 11:50 am.. Common Area – LPA observed the living room, dining room, and kitchen are combined. The living room area contains comfortable seating. The dining room area contains dining room table and chairs, The medications will be locked in a cabinet within the pantry area. The cabinet underneath the kitchen sink was locked and contained cleaning solutions. There is a locked drawer for knives. A fire extinguisher is also located in the kitchen. Backyard - The backyard is shaded and contains comfortable seating. The gate leading from the back yard to the front yard was not locked. Bedrooms - The bedrooms contained bed, linens, night stand, lamp, chest of drawers, and a chair. Bathrooms - There are three bathrooms which contains walk in showers, hand soap, paper towels, trash can, grab bars, and slip resistant mats. Continued on 809-C Water Temperature - LPA observed the water temperature at 11:20 am and the temperature was 106.0 degrees F. Staff Room - LPA observed the staff room which was locked. Smoke/Carbon Monoxide Detectors – The detector was tested at 11:30 am and was operable. Auditory Alarm - LPA observed the alarms on all the doors were properly working Resident Records – LPA reviewed the resident records from 12:00 pm until 12:30 pm. Staff Records - LPA reviewed the staff records from 12:30 pm until 12:50 pm. LPA Spaeth observed one of the two staff members present at the facility has not received the fingerprint clearance notification letter. During LPA's visit, LPA observed the staff member left the facility. Medication - LPA observed the resident medications and did not see any issues. Based upon LPA's observation, the following deficiency was cited. Exit interview, appeal rights discussed, and a copy of the report was given.
