What the inspector wrote

On 3/30/26 Licensing Program Analyst (LPA) Perchui Milena Khurshudyan conducted a Case Management visit at the facility. Upon arrival, LPA met with the Caregiver who granted access to the facility. LPA introduced herself by showing the department badge and explained the reason for the visit. The Administrator, Ilanit Asseraf got contacted over the phone and arrived shortly after. The purpose of today’s visit is to evaluate the facility’s request to increase the approval of Hospice Waiver capacity from two (2) to six (6) residents. During today’s visit, LPA conducted a physical plant tour of the facility, observed overall operation, and assessed whether the facility is able to safely accommodate additional residents receiving hospice services. LPA observed the facility to be clean, organized, and in good repair. Residents’ rooms, common areas, bathrooms, kitchen, and outdoor areas were observed to be in a safe and comfortable manner, exits were free from obstructions. LPA also observed staff providing care and supervision to residents. Staff were observed to be professional and responsive to residents’ needs. Residents appeared comfortable, happy, and staff interacted with residents in a respectful manner. LPA interviewed residents who stated they are satisfied with the care and supervision provided at the facility. Residents did not express any concern regarding staff support and daily care. LPA also interviewed staff regarding daily duties and supervision of residents on hospice and understanding of their care needs. LPA also reviewed staff training records and determined that staff have completed all required training courses. Continue on LIC809-C Based on today’s visit and LPA’s observations, the facility request to increase the hospice waiver from two (2) to six (6) residents will be granted. No Deficiency issued during today’s visit. Exit interview conducted, a copy of this report signed and delivered.
