What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted a Pre-Licensing Inspection with the applicant representative/administrator, Narineh Ohanian. An Application to operate a Residential Care Facility for the Elderly (RCFE) was received by Community Care Licensing (CCL) on November 6, 2024. A fire clearance was approved on February 20, 2025 for five (5) non-ambulatory residents and one (1) bedridden resident, for a total capacity of six. The applicant is also requesting a hospice waiver to retain six (6) residents. The smoke alarms and carbon monoxide detector are dual and inter-connected. The facility has one new fire extinguisher, located by the kitchen. It was was purchased on February 11, 2025. A tour of the physical plant was initiated at approximately 9:50am and the following was observed: KITCHEN: The facility has a Kitchen area that is equipped with a refrigerator, stove/oven, microwave oven and sink. There were adequate supplies of perishable and nonperishable food and dining ware to accommodate a maximum capacity of six (6). Knives were observed locked in a kitchen drawer. BEDROOMS: There are four (4) bedrooms designated for client use. All four bedrooms can be shared, with bedroom #3 having the bedridden fire clearance (per STD 850). The applicant furnished the resident bedrooms with beds, night stand, chairs, dresser, bedding and linen. Each bedrooms have sufficient lighting and closet space. Additional linen and towels are kept in the linen closet, out in the hallway BATHROOMS: The facility has three (3) bathrooms. Bedroom #4 has it's own bathroom with shower. There is a full bath in the hallway, next to bedroom #1. There is another full bath in between rooms #2 and #3. All bathrooms were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water delivered in the bathrooms measured at 116 degrees. COMMON AREAS: These included the living and dining room. The living room is furnished with a large and small couch and coffee table. The dining room table was large enough to seat six (6) individuals. There is a television placed by the dining room, with an activity table with board games underneath. There is a fireplace with a screen and a glass slide. No key or fireplace fixtures present. There is a sign in table by the dining room table, where the first aid and first aid manual is kept. LAUNDRY ROOM: The laundry room is located adjacent to the first common bathroom in the hallway. It will be kept locked at all times as cleaning supplies and detergents will be maintained there. MEDICATIONS: The medication closet is located near the front entrance. It has a lock. Staff and resident records will also be maintained there. OFFICE/STAFF WORKSTATION: Staff Office/Workstation is located between rooms #2 and #3. GARAGE: No garage, car port only. SURROUNDING GROUNDS: The driveway, passageways and entrance to the home was clear of obstruction. All entry and exit doors have a functional auditory alert when the doors open. The backyard of the facility has a patio and backyard furniture to accommodate the six (6) residents. The facility backyard has sufficient yard space. There is no swimming pool or any other bodies of water. There are two storage sheds that will be locked and used to keep supplies. Side gate/exit is clear of obstruction. In addition to the Pre-Licensing inspection, a Component III power point presentation was also held. Pursuant to Title 22, Division 6 of the CA Code of Regulations, the facility's physical environment is compliant and ready for licensure. CAB will be advised and a copy of this report provided.
