What the inspector wrote

An informal meeting was held today at the Woodland Hills Regional Office to discuss recent deficiencies and provide guidance to ensure future compliance. Prior to the meeting, Licensee was given the chance to review the facility file. Present at today's meeting were the following: Sergey Azaryan - Administrator Nichelle Gillyard - Licensing Program Manager (LPM) Angela Panushkina - Licensing Program Analyst (LPA) The informal conference process was explained to the Licensee. The Licensee was also informed that this Informal Meeting can lead to an Administrative Action Process. BRIEF HISTORY: The facility Aurora Lemarsh has been in operation since licensure on 06/13/2025. From June 2025 to present, the Department received one (1) complaint, which was found Unsubstantiated. Continue on LIC809-C Additionally, a Case Management - Deficiency visit was also conducted in conjunction with a complaint control #31-AS-20260203081859. During the visit LPA was informed that the facility had seven (7) residents. LPA conducted physical plant tour and confirmed that there were seven (7) residents present. LPA conducted review of facility license, issued on 06/13/2025, and observed that the facility had been approved for a maximum capacity of six (6) residents. Deficiency and Civil Penalty of $500.00 was issued. LPA conducted review of the Licensing Information System (LIS) and observed that the Administrator is associated with the following facilities: Aurora Senior Living (LIC# 197610692) which was licensed on 02/07/25. The Administrator informed LPA and LPM that the facility is currently going through the Change of Ownership (CHOW), and during the entire transition the Administrator will be fully responsible for following all Title 22 Regulation and be in compliance. Aurora Raymer (LIC#197610915 - pending). Regional Office received an Unlicensed complaint (control #31-AS-20260202141503). On 02/09/26, the Department conducted an initial visit and based on their interviews and observation it was determined that two (2) out of the two (2) individuals residing in the home required care and supervision. Therefore, the complaint was found Substantiated. During todays meeting, all citations were discussed and the Department was informed that the facility currently has six (6) residents and all staff had been trained. Administrator, assured the Department that there will be no citations moving forward. Lastly, the Department requested current LIC500 (Personnel Report) to be submitted on or before 03/25/2026. Also, the Administrator will be referred to the Technical Support Program (TSP). Exit interview conducted and copy of this report signed and delivered.
