What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted a Pre-Licensing Inspection with the applicant representative, Byuzand Berberyan and administrator, Hayarpi Berberyan. An Application to operate a Residential Care Facility for the Elderly (RCFE) was received by Community Care Licensing (CCL) on December 6, 2024. The facility is a one story building. A fire clearance was approved on May 5, 2025 for five (5) non-ambulatory residents and one (1) bedridden resident, for a total capacity of six. The applicant is also requesting a hospice waiver to retain six (6) residents. The smoke alarms and carbon monoxide detector are dual and inter-connected. The facility has one new fire extinguisher that was purchased on April 10, 2025. It is located at the kitchen. A tour of the physical plant was initiated at approximately 9:45am and the following was observed: KITCHEN: The Kitchen is equipped with a refrigerator, microwave, stove/oven and sink. There is an adequate supply of nonperishable food and dining ware to accommodate a maximum capacity of six (6). Perishable items not needed at this time as facility is vacant/no residents. Applicant and administrator was advised they need to supply perishable food items once they start admitting residents in care. Knives were observed locked in a kitchen drawer. There is a locked cabinet where medications will be stored. BEDROOMS: There are four (4) bedrooms designated for client use. Bedroom #1 and #3 are shared rooms, with bedroom #1 having a bedridden fire clearance (per STD 850). In addition, Bedroom #1 has it's own bathroom. Bedrooms #2 and #4 are private rooms. The applicant furnished the resident bedrooms with beds, night stand, chairs, dresser, bedding and linen. The bedrooms have sufficient lighting and closet space. BATHROOMS: The facility has two and one half (2 1/2) bathrooms. Bedroom #1 has it's own bathroom with shower. The other bathroom is located in the hallway. The half bathroom, located near the kitchen, is for staff use only. The bathrooms were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water delivered in the bathrooms measured between 105 to 107 degrees. COMMON AREAS: These included living room and dining room. The living room is properly furnished with appropriate seating, chairs, table and television. The fireplace is properly screened. It is functional, but key is kept locked. No tools present. The dining area has a large dining room table to accommodate six (6). Floors and furniture were clean and in good repair. GARAGE/ LAUNDRY ROOM: The garage is attached to the house. The laundry area is located in the garage. Cleaning supplies and detergents will be kept in a locked cabinet there. MEDICATIONS: The medications will be kept locked in a kitchen cabinet with first aid kit and manual. OFFICE/STAFF WORKSTATION: Staff office/workstation is located by the dining room. Resident and staff records will be kept in a locked cabinet there. SURROUNDING GROUNDS: The driveway, passageways and entrance to the home were clear of any hazard. All entry and exit doors have a functional auditory alert when the doors open. The backyard of the facility has a patio and backyard furniture to accommodate the six (6) residents. The facility backyard has sufficient yard space. There is no swimming pool or any other bodies of water. Side gates were checked to insure exits are free of any obstruction. In addition to the Pre-Licensing inspection, a Component III power point presentation was also held. Pursuant to Title 22, Division 6 of the CA Code of Regulations, the facility's physical environment is compliant and ready for licensure. CAB will be advised and a copy of this report provided.
