Pre-licensing visit·March 27, 2026

At 10:30 am Licensing Program Analyst (LPA) Tihesha Smith made a subsequent announced visit to this facility. LPA was greeted by the Licensee and administrator. The purpose of the visit was to verify that the items observed during pre-licensing visit have been corrected. The licensee and administrator confirmed updated sketch with room changes was sent to CAB specialist. Slips mats are in the showers. The bedroom sleeping arrangement in closet has been removed. Individual who licensee is POA for has moved from the home and will not be a resident. The installed grab bars were found to be unstable and/or inadequate to provide safe support for handholds during maneuvering to reduce the risk of falls. Remaining items: · Bedroom #1 Bathroom: need secure grab bars · Bedroom #2 bathroom: need secure grab bars · Bedroom #3 bathroom: need secure grab bars · Guest bathroom: need secure grab bars Licensee/Administrator will provide pictures to LPA Smith once installed. Garage observed to be used for storage Copy of this report will be forwarded to the CAB specialist. Exit interview conducted/Copy of report given.
