What the inspector wrote

Licensing Program Analysts (LPAs) Evelin Rios and Lorena Casillas conducted a Prelicensing inspection visit to this facility. This is for a Change in Ownership (CHOW) application. LPA met with the Executive Director (ED) and Applicant Representative, Kortnie Spitznogle and explained to her the purpose of the visit. At approximately 10:00 a.m., LPAs requested the residents roster, Personnel Report (LIC500), staff and residents records. LPA's reviewed ten (10) staff files and ten (10) resident records. At approximately 2:00 p.m., LPA Rios initiated a tour of the facility inside and out. The facility is a two story building with a memory care unit that is equipped with delayed egress. There is one (1) entrance being utilized at the facility. Guests and visitors are asked to sign in at reception. The facility has submitted an approved Infection Control Plan and Emergency Disaster Plan LIC610E. According to the ED the facility will carry over any previous admissions agreement to the new license. The independent living unit has two laundry rooms accessible to residents, as well as a larger staff laundry room that also stores detergents and other cleaning supplies. LPA also observed a kitchen, visitor and resident bathrooms, an activity room and dining area. LPA observed a beauty salon on the second floor. LPA observed common rooms to be appropriately furnished, clean and in proper order. The facility has three stairwells with an emergency chair on each and a functioning elevator. The memory care unit has a delayed egress, which was tested. The memory care unit has it's own dining room and living area, which was observed to be appropriately furnished, clean and in proper order. (Continue to LIC809-C) A fire inspection for the fire alarms was conducted on 02/13/2026 by Cintas and facility passed inspection. The facility is equipped with sprinkler system. Fire extinguishers are located throughout the facility. The facility was granted their fire clearance on 10/22/25. Personal accommodations in resident bedrooms and bathrooms were observed for safety, privacy, and comfort. Random resident rooms were inspected in the independent living unit and memory care unit. Bedrooms were observed with all required furnishings, working signal system, grab bars and nonskid surfaces in the bathrooms. Hot water temperature in random resident bathrooms were checked and measured a range of 115.2°F to 118.4°F. Medications were observed locked in two medication carts located in each unit. Both units also have medication rooms that were observed locked and inaccessible to residents. There are three complete first aid kits on site. Component III was completed. Pre-Licensing is complete and this facility has no deficiencies. No health and safety hazard were noted during this visit. Exit interview was conducted and a copy of report was issued.
