At 9:30am, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced case management visit to this home. Upon arrival, LPA met with the Administrator and explained the reason for the visit. On 03/21/2025 an application for licensure was submitted. While the applicant requested a Fire Clearance for five (5) Non-Ambulatory and one (1) Bedridden residents, it was discovered (on 08/15/25) that this address (20601 Mayall St., Chatsworth, CA 91311) was under investigation with Los Angeles Department of Building and Safety (LADBS). Thus, the Fire Clearance was placed on hold. On 09/09/25, LADBS conducted their final inspection, and all pending permits were approved and submitted to the Regional Office (RO). During today’s visit, LPA was informed that a Fire Clearance was already requested, and the Administrator is currently waiting for the Component II which was already scheduled for 09/15/2025 at 11:00am. At 10:10am, LPA conducted a tour of the facility and observed the home to be clean and have sufficient food supply. LPA also confirmed that the home continues to provide care and supervision to four (4) residents. Per Administrator R1 passed away on 08/16/25 and a notification was submitted to the RO and no additional incidents with other residents had happened since. LPA reminded to Administrator that the application for Residential Care Facility for the Elderly (RCFE) license for this location is currently pending. LPA also reminded the Administrator that no new residents shall be admitted at this time due to the pending status. Exit interview conducted and copy of this report signed and delivered.
