What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted a Pre-Licensing Inspection with the applicant/administrator, Gayane Manukyan, and her consultant, Ripa Tavitian. An Application to operate a Residential Care Facility for the Elderly (RCFE) was received by Community Care Licensing (CCL) on June 18, 2025. The facility is a one story building. A fire clearance was approved on July 7, 2025 for five (5) non-ambulatory residents and one (1) bedridden resident, for a total capacity of six. The applicant is also requesting a hospice waiver to retain six (6) residents. The smoke alarms are hard wired and inter-connected. The carbon monoxide detector is functional and installed in the hallway. The fire extinguishers is located by the entrance. It was purchased on June 21, 2025. A tour of the physical plant was initiated at approximately 10:00am and the following was observed: KITCHEN: The kitchen area is equipped with a refrigerator, stove/oven, and sink. At this time, perishable food items not required as there are zero (0) residents. Applicant advised to purchase perishable food items once facility is licensed and operational. There is an adequate supply of nonperishable/emergency food supply. BEDROOMS: There are four (4) bedrooms designated for resident use. Bedroom #1 and #2 are private. Bedrooms #3 and #4 are shared. Bedroom #1 is approved for bedridden (per STD 850). The applicant furnished the resident bedrooms with beds, night stand, chairs, dresser, bedding and linen. The bedrooms have sufficient lighting and closet space. BATHROOMS: The facility has three (3) bathrooms. Two (2) bathrooms are designated for resident use, and one for staff. Bedroom #2 has it's own bathroom. The bathrooms designated for resident use were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water in resident bathrooms were measured between 108 to 111 degrees. COMMON AREAS: These included the living room and dining room. The living room is furnished with adequate seating, couches, table and television. The dining area has a table large enough to seat up to six (6) residents. There is no fireplace. Floors were clean and in good repair. Passageways and hallways were clear of obstruction. LAUNDRY: The laundry area is located at the side of the home. MEDICATIONS: The medication cabinet is located by the dining room. LPA observed storage bins for resident use. A complete first aid kit and manual is also maintained in the medication cabinet. GARAGE: There is no garage. SURROUNDING GROUNDS: The driveway, passageways and entrance to the home were clear of obstruction. All entry and exit doors have a functional auditory alert when the doors open. The backyard of the facility has a patio and backyard furniture to accommodate the six (6) residents. The facility backyard has sufficient yard space. There is no swimming pool or any other bodies of water. In addition to the Pre-Licensing inspection, a Component III power point presentation was held at the start of the visit. Pursuant to Title 22, Division 6 of the CA Code of Regulations, the facility's physical environment is compliant and ready for licensure. CAB will be advised and a copy of this report provided.
