What the inspector wrote

At approximately 1:00 p.m. on 12/30/25, Licensing Program Analyst (LPA) Nicholas Reed conducted an announced prelicensing inspection. LPA met with the administrator and applicants and disclosed the reason for the visit. Today’s prelicensing inspection is an initial application without residents in care. It is a single story building with four (04) bedrooms, three (03) bathrooms, kitchen, laundry room, common areas, and outdoor areas. It has an approved fire clearance for one (01) ambulatory resident in Bedroom #1 and five (05) nonambulatory residents, of which one (01) may be bedridden in Bedroom #4. Approved hospice waivers for six (06). LPA observed a maintained front yard with new rails and ramps leading to the main entrance. Postings at the front and in the hallway included COVID precautions, facility license, confidential complaints contacts, emergency disaster plan, rights of resident councils, theft and loss policy, personal rights, and the facility sketch. Walls, floors, windows, screens, and blinds were clean and in good repair. At 1:10 p.m. LPA measured the room temperature to be 68 degrees Fahrenheit. The living room contained a television, reading material, exercise equipment, puzzles, art supplies, and furniture in good repair. A fireplace was appropriately covered and turned off. Nightlights were observed in common areas and walkways. The staff bathroom was clean and odorless. At 1:15 p.m. LPA called out from the house telephone which was deemed operational. WiFi, a tablet, and a computer were available for residents. At 1:20 p.m. the room temperature was measured to be 68 degrees Fahreheit. LPA observed an adequate supply of perishable and non-perishable foods in the kitchen. Appliances were in good condition. Sharps were locked above the counter. The stove surface and hood were clean. A washing machine and dryer were located in the laundry area near the kitchen. Both were in working order. Detergents and cleaning supplies were locked near the appliances. Medications were locked separately nearby. Emergency food and water supplies were stored nearby but separately. The facility has five (05) bedrooms. All bedrooms contained a chair, lamp, nightstand, storage, bell or call system, and a bed with adequate bedding. All furnishings were clean and in good condition. All rooms had hospital beds with wheels in the locked position. The facility has three (03) bathrooms. One (01) bathroom is private to Bedroom #4 and one (01) is shared. All bathrooms contained liquid soap, paper towels, trash can with a tight fitting lid, grab bars near the toilet and shower, commodes, and a non-skid mat in the shower. At approximately 1:30 p.m. LPA measured the water temperature in the shared bathroom to be 105.3 degrees Fahrenheit. LPA observed a shaded patio area in the rear of the facility. The patio contained furniture in good condition. Fruit-bearing trees were maintained. Two (02) locked sheds contained extra supplies. The emergency exit paths was free from obstructions. The exit gate was unlocked. Three (03) out of three (03) auditory alarms were turned on and functioning. At 1:45 p.m., smoke and carbon monoxide detectors were tested and operational. At approximately 1:55 p.m. LPA observed a fully charged fire extinguisher in the kitchen. It was purchased on 12/01/2025 with a receipt attached. At 2:15 p.m., LPA and the applicants reviewed COMP III. During today's inspection, the facility was in compliance with Title 22 regulations. No immediate health or safety hazards were observed. The facility is ready for licensure. Exit interview conducted. Copy of report provided.
