What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted a Pre-Licensing Inspection with the administrator, Karo Berberyan and house manager, Nelli Berberyan. An Application to operate a Residential Care Facility for the Elderly (RCFE) was received by Community Care Licensing (CCL) on October 6, 2025. A fire clearance was approved on December 5, 2025 for five (5) non-ambulatory residents and one (1) bedridden resident, for a total capacity of six (6). The applicant is also requesting a hospice waiver to retain six (6) residents. The smoke alarms and carbon monoxide detector are dual, hard wired and inter-connected. The facility has two new fire extinguishers that was purchased on January 8, 2025. One fire extinguisher is located by the kitchen, and the other at the entrance. Prior to the Pre-Licensing, a Component III presentation was held. A tour of the physical plant was initiated at approximately 10:00am. The facility is a one story building. The following was observed: KITCHEN: The kitchen area is equipped with a refrigerator, stove/oven, microwave and sink. At this time, perishable food items not required as there are zero (0) residents. Applicant advised to purchase perishable food items once facility is licensed and operational. There is an adequate supply of nonperishable food and dining ware to accommodate a maximum capacity of six (6). Knives and sharps will be locked in a kitchen drawer. BEDROOMS: There are five (5) bedrooms designated for client use. Bedroom #1 is shared. Per STD 850, Bedroom #1 has the bedridden fire clearance. Bedrooms #2, #3, #4 and #5 are private. The applicant furnished the resident bedrooms with beds, night stand, chairs, dresser, bedding and linen. The bedrooms have sufficient lighting and closet space. BATHROOMS: The facility has two (2) full bathrooms, and one (1) half, reserved for guest and staff. Bedroom #2 has it's own bathroom with shower. The other two bathrooms are located in the hallway. The bathrooms designated for resident use were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water delivered in the bathrooms were measured between 108 to 110 degrees. COMMON AREAS: These include the living room and dining room area. The living room is furnished with sufficient seating, couches, chair and table. The dining room table can seat up to six (6) individuals. There is a television installed on the wall by the dining area. There is a fireplace with a screen and a glass slide. It is non-operational, with the key stored and inaccessible to residents. No fireplace tools or fixtures present. LPA observed magazines and board games for activities. LAUNDRY ROOM: The laundry room is located by the kitchen. LPA observed a brand new washer and dryer. Detergents, softeners, and cleaning supplies will be kept in a locked cabinet above the washer and dryer. MEDICATIONS: The medication cabinet is located in the hallway. LPA observed a brand new first aid kit and manual. Medication cabinet will be locked at all time. OFFICE/STAFF WORKSTATION: Staff office is located at the entrance. Staff and resident records will be maintained there. GARAGE: The garage is attached to the house. Entry way is through the laundry area. Garage will be used as storage. Garage is kept locked, and requires a key for access. SURROUNDING GROUNDS: The driveway, passageways and entrance to the home was clear of obstruction. All entry and exit doors have a functional auditory alert when the doors open. The backyard of the facility has a patio and backyard furniture to accommodate the six (6) residents. The facility backyard has sufficient yard space. There is no swimming pool or any other bodies of water. Pursuant to Title 22, Division 6 of the CA Code of Regulations, the facility's physical environment is compliant and ready for licensure. CAB will be advised and a copy of this report provided.
