Pre-licensing visit·March 18, 2026

At 10:00 am, Licensing Program Analyst (LPA) Huma Rahimi conducted a Pre-Licensing Inspection with the facility Administrator, Gohar Artemyan. This is a Change of Ownership Application from facility # 197610444 to # 197610955 to operate a Residential Care Facility for Elderly (RCFE). A Change in Ownership (CHOW) Application to operate a RCFE was received by Community Care Licensing (CCL) on October 27 th , 2025. A fire clearance was approved on November 14 th , 2025, for five (5) Non-ambulatory residents, and one bedridden resident in room #4. A tour of the physical plant was initiated at approximately 10:10am and the following was observed: KITCHEN: The facility has a Kitchen area that is equipped with a refrigerator, gas stove, microwave oven and sink. There were adequate supplies of two (2) days of perishable and seven (7) days of nonperishable food, dining ware to accommodate a maximum capacity of six (6) residents. Knives and other sharps were observed locked in one of the kitchen cabinets next to the fridge. One of the fire extinguishers was located on the countertop in the kitchen and another one was observed hanging on the wall by the dining area and were last purchased on 03/15/26. MEDICATIONS: The medications are kept in a cabinet located in the kitchen entrance. At approximately 10:12 am LPA observed each centrally stored prescription and PRN medication has been logged in the medications log with proper documentation from the residents' doctor. All medications are properly labeled and checked for expiration dates. LPA observed a First Aid and the new manual on the counter of the living room. The First aid has all proper items and is current. Continue on LIC809-C LAUNDRY ROOM: The washer and dryer is in the garage of the facility. The washer and dryer are in good working condition. LPA observed all chemicals are kept locked and inaccessible to residents in care in the garage cabinet. BEDROOMS: The facility has four (4) bedrooms, and all four (4) bedrooms are designated for residents' use. Bedroom #1 is private and bedrooms #2 and #3 are shared. Bedroom #4 is a bedridden and is private. The applicant furnished all clients' bedrooms with beds, nightstands, chairs, dresser, bedding and linen. The bedrooms have sufficient lighting and closet space. Facility has an awake staff. BATHROOMS: The facility has two (2) bathrooms and were observed to have the proper fixtures, grab bars, non-skid mats, trash cans with closed lids. The hot water delivered in the bathrooms measured at 10:15 am to be at 119.8 degrees. COMMON AREAS: These included living room and a dining room. The living room was equipped with furniture, a television, tables and chairs. The fireplace was secured with a screen. The dining area has a dining room table to accommodate six (6) residents, a sofa and an office area in the corner of the living room. There were no visible immediate hazards. The facility has comfortable temperature of 77 degrees. At 10:20am, hard wired and inter-connected smoke alarms and carbon monoxide detectors were tested and observed operational. SURROUNDING GROUNDS: The driveway, passageways and entrance to the home were clear of obstruction. The backyard of the facility is properly fenced, has a patio and backyard furniture to accommodate six (6) residents. The facility has a swimming pool; however, the applicant covered it and is not operational. LPA observed that the swimming pool area is properly fenced during the visit. RESIDENT/STAFF FILES : From 11:25 am to 12:45 PM, LPA conducted a file review of six (6) residents and two (2) staff and observed all files completed and updated. In addition to the Pre-Licensing inspection, a Component III power point presentation was also held. Pursuant to Title 22, CA Code of Regulations, the facility's physical environment appears to be compliant and ready for licensure. CAB will be advised. Exit interview conducted and copy of this report signed and delivered to the Administrator.
