Pre-licensing visit·March 19, 2026

Licensing Program Analyst (LPA) Tihesha Smith conducted an announced pre-licensing visit at this facility at 9:30 am. The licensee and administrators’ ( Suren and Vahagn Sargsyan) identity were verified by CA photo ID. The facility has a capacity of six (6). Purpose of today’s visit is to inspect the facility to ensure that the facility is following the rules and regulations of California Code of Regulations, Title 22, Division 6. The facility is a single-story building. Today's site visit consisted of LPA Smith touring the physical plant inside and outside and the following was observed: The common areas (kitchen, living room, and dining) areas were appropriately furnished, and lighting was adequate. The facility has some perishable and non-perishable food supply with emergency water in extra space near the inside garage door. Appliances in the kitchen appeared to be functional. The living room has a television and comfortable furniture. Games and first aid kit are stored on table in dining room. The sharps are stored and locked in kitchen drawer. The medications are locked in kitchen top cabinet near window and toxins locked under kitchen sink. Laundry area is accessed through kitchen. The laundry appliances observed to be in sufficient repair. There is one (1) fire extinguisher: attached to kitchen wall and was observed to be fully charged. Dual Smoke and Carbon Monoxide detectors were observed all over the facility, tested, and observed to be operational at time of visit. Emergency exit plan/sketch is posted near the entrances and in the bedrooms with other posting requirements on dining area wall. (cont from 809) The bedroom show room furnihed with required furniture: nightstand, lamp, dresser, and chair. All rooms will have same furnishings. LPA Smith discussed benefit of separate dresser versus shared dresser with licensee and administrator. Extra linens are stored in hall closet. There are two (2) bathrooms in the facility with one (1) of the bathroom designated for residents’ use. Bathrooms have trash cans, non-slip mats and grab bars. Bathroom water temperature measured at: 110 degrees Fahrenheit. Staff and residents’ files stored in cabinet in staff room/office. Backyard has a covered patio area with table, sufficient seating and is in good repair. Garage: to be used for facility storage only if part of facility/personal items need to be removed Back area side of garage: loose bricks/small cement to be removed Component III presentation conducted at 10:20 a.m. and upon completion the administrator acknowledged understanding. The facility will not have a bedroom designated for staff and extra space between laundry area and garage will not be used as a bedroom at anytime. At this time the facility is meeting licensing requirements. Copy of this report will be forwarded to the CAB specialist. Exit interview was conducted and a copy of report was issued.
