In regards to the allegation: Facility staff did not notice a change in the resident's condition. Based on the review of Medical Records (HouseCall MD, Inc.) which indicate from 12/12/19 through 06/04/21, in house medical services were provided to Resident #1 on a monthly or as needed basis. Medical Records documented Resident #1's medical history and assessment plan, therefore facility was aware of any changes in Resident #1's medical conditions. Based on LPA's record review, investigation revealed: Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Exit interview conducted with Martha Rosas (Reception) and a copy of this report provided.
