What the inspector wrote

License Program Analysts (LPA) Luis De Leon conducted an unannounced annual required visit. LPAs met with Assistant Celina Vasquez. The purpose of today’s visit was explained to facility personnel met during this visit. Facility is licensed to serve six (6) ambulatory and non-ambulatory residents aged 60 and above of which 1 may be bedridden. Facility is approved to care for dementia resident. Manager Cynthia Tadeo arrived later and joined the visit. The LPA use the Compliance & Regulatory Enforcement Tool (CARE) during today’s inspection. The visit consisted as follows: Ms. Tadeo informed LPA that there are currently no residents residing in Walnut Villa - Telegraph I and informed LPA that she is currently helping with the transition to a new ownership. The property lot is made up of three (3) separate buildings (13971, 13973, & 13975 Telegraph Road). LPA toured the facility at 13971 and observed that all interior rooms are under construction. The facility has three (3) bedrooms, one bathroom, office, a nook, kitchen, living room, and dining room. LPA toured the house and did not observe any residents living in the facility. Ms. Tadeo informed LPA that a letter was sent to Monterey Park CCLD office informing of the remodeling project to bring facility to compliance and ready to accept residents. New ownership has started the process to obtain a new license for one of the buildings. LPA explained to Ms. Tadeo that, if the new ownership plans to keep each building with its own individual license, the new ownership needs to apply for a new license for each building. LPA observed that new license process has been started for building 13975 only. No deficiencies cited. An exit interview was conducted, and a copy of this report was provided to ManagerTadeo.
