What the inspector wrote

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced required 1-year visit on 03/16/2026. LPA was met by staff Celina Vasquez and explained the purpose of the visit. Facility is licensed to serve six (6) ambulatory and non-ambulatory residents aged 60 and above of which one (1) may be bedridden. Facility is approved to care for dementia residents. The property lot is made up of three (3) separate buildings (13971, 13973, & 13975 Telegraph Road). LPA OBSERVATIONS: The facility is a single-story dwelling located on a residential street and consists of three (3) resident bedrooms, one (1) bathroom, office, kitchen, dining room, living room, detached garage, and backyard. LPA used the Compliance & Regulatory Enforcement Tool (CARE) during today’s inspection. LPA observed this facility to be under construction, and zero (0) residents and zero (0) staff were observed inside the facility. According to staff Vasquez, this facility has been under renovation since 2022 and has not had any residents or staff in the facility. During file review, LPA Ramirez observed documentation sent to Monterey Park Regional Office that indicates no residents will be in the facility until all renovations are completed and licensing is notified. LPA observed construction materials on the inside and outside of the facility. Per staff Vasquez, no staff are assigned to this facility, therefore LPA did not review staff files or resident files. All annual fees are current for this facility. Staff Vasquez agreed to submit facility Emergency Disaster Plan for Residential Care Facilities for the Elderly LIC (610E) and liability insurance upon admission of the first resident. No deficiencies were cited during this visit. Exit interview was conducted and a copy of this report was provided.
