What the inspector wrote

On 09/11/25 at 9:10 AM, Licensing Program Analyst (LPA) Mario Leon conducted an unannounced required annual inspection and met with Oscar Lechuga - Administrator. This facility is licensed to serve 6 adults ages 60 and above, of which 4 may be non-ambulatory residents. Facility must maintain a 24-hour awake staff. Facility is approved for 6 hospice residents. The facility currently has no resident(s) on hospice. The facility is a one-story facility located on a residential street. The house consists of 3 resident bedrooms, 1 staff bedroom, 2 bathrooms, 1 dining/living room, 1 kitchen, 1 attached garage, and a front yard and backyard patio area, both yards presented with shaded seating. LPA toured inside and outside grounds and no bodies of water were observed. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident. Each resident resides in a private room and there are no weapons on the premises. Resident bathrooms were checked, toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and non-skid mats were in place. Screens in bathroom number one (#1) and bathroom number two (#2) both need screens replaced to remain within regulation, see LIC809-D.. Hot water average temperature was measured at 106.2 °F (degrees F) and all hot water reached at least 105°F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked within each bathroom with additional items stored in the hallway closet. Common areas were clean and clear of hazards and doorways and emergency exit(s) were free of obstructions. LPA toured the kitchen area and observed a two-day supply of perishable and a seven-day supply of non-perishable food. One burner of the stove was in disrepair, please see LIC809-D. Knives were kept in locked toolbox and cleaning detergents are stored not accessible to residents in care. First-Aid kit was available and two (2) manuals were present. One fire extinguisher, last serviced January 13, 2025 was observed in the kitchen area. Staff tested the carbon monoxide detector and smoke detectors in the house. Both devices were functional. An exit interview was held with Oscar Lechuga - Administrator and a copy of this report, appeal rights and deficiencies cited have been provided.
