What the inspector wrote

On 06/18/2025, Licensing Program Analyst (LPA) Jose Anguiano conducted an unannounced annual required visit. LPA met with Lead Staff Kendra Howard and explained the purpose of today’s visit. The facility is licensed for 4 age ambulatory elderly ranging 60 and over with a hospice waiver for 3 residents. A total of 4 clients are currently enrolled in this facility. The facility consists of the following: One story building, 4 bedrooms, 2 living room, 3-bathroom, 1 kitchen area, 1 office, 1 laundry and a backyard patio area with shaded seating. LPA toured the physical plant. There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident's personal belongings was observed. Bed linens, comforters, and bath towels were stocked during the visit. A comfortable temperature was maintained in the facility at 75 degrees F. LPA observed the facility to be furnished at the time of the visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected, and 2 days supplies perishable, and 7 days non-perishable food was maintained. Smoke detectors and carbon monoxide were operable. LPA observed screening protocols for visitors, staff, and residents, and sanitizing stations in common areas and restrooms. All mandated inspection control posters were posted including Food Menu. No deficiencies were cited during today’s visit. An exit interview was conducted with lead staff and a copy of the report was provided. Due to time constraints, only a portion of the visits was completed. The remaining items will be addressed upon return to the facility.
