What the inspector wrote

On 05/08/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced visit to the above facility to conduct an annual inspection. LPA met with Tana Lord as the purpose of the visit was explained. Facility is licensed to serve (6) non- ambulatory residents age 60 and over of which (1) maybe bedridden. The facility has an approved hospice waiver for (6). Facility fees are current, liability insurance is active (Kinsale Insurance Co. 0100245465-2 Exp: 06/19/26). This home is a single story home consisting of: (6) resident bedrooms, (3) full bathroom, a den, living room, kitchen with dining area, detached garage that serves as a laundry area, and an outdoor shaded patio area. Bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to residents. The water temperature properly measured between 105-120 F.. A supply of perishable and non-perishable food was observed, toxins and knifes were stored and inaccessible to residents, no weapons nor bodies of water on the premises, exits and walkways are free of debris/hazards. LPA conducted a records review of 4 staff records, 4 resident records, and 4 medication administration records, no discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire was conducted on 03/22/26, carbon monoxide and smoke detectors are interconnected and operational. (3) Fire Extinguishers were checked and found to be fully charged and accessible. Long Beach fire life safety inspection conducted on 11/27/25. All exit doors and windows in the facility have alarm systems. Exit interview conducted, and a copy of this report was provided.
