LPA conducted interviews with resident 1-5 (S1-S5) and 5 out of 5 residents stated that their medications are given to them daily. LPA also conducted interviews with staff members 1-3 (S1-S3) and 3 of 3 stated that residents are given their medications as prescribed by their physicians. LPA also observed the medications and list for R1-R5 and it appears that the residents are receiving their medications as prescribed by their physicians. LPA also toured the facility and did not observe any health or safety concerns Based on interviews, file review and observation during the investigation, the above allegation are found to be Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. An exit interview was conducted where this report was discussed and provided to Natalya Trakhtenberg Administrator at the conclusion of the visit with appeal rights
