On 11/03/2021 Licensing Program Analyst (LPA) Don Senaha conducted a Case Management visit to follow up on an incident report. LPA was greeted by Administrator Kadiguia Linayao and explained the purpose of the visit was to get more details about the incident reports. LPA met with Administrator Kadiguia Linayao and discussed the incident reports on 10/29/2021 and 10/30/2021. LPA gathered contact information for staff. LPA interviewed staff (S1, S4) about incidents. LPA did a tour of the physical plant. There were two (2) staff and no residents present at the time of the visit. There were no deficiencies found at the time of the visit. An exit interview was conducted with Kadiguia Linayao and a hard copy was provided for signature.
