What the inspector wrote

A Non-Compliance Conference (NCC) was held at the El Segundo Adult and Senior Care Program Regional Office on 4/22/26. Participants in this meeting included the following: Janae Hammond, Interim Regional Manager; Alfonso Iniguez, Licensing Program Analyst; Ernand Dabuet, Licensing Program Manager; and Charesa Reyes/Licensee. The purpose of this conference is to discuss the multiple substantiated unlicensed complaint investigations by the department. Based on the complaint investigations listed below, the Operator provided elements of care and supervision to residents that should be in licensed assisted living facilities. The Licensees stated the following plans in order to bring the facility into compliance: · Licensee will cease, apply for a license, or relocate residents who require elements of care and supervision from all unlicensed properties by 05/08/2026. · The licensee will provide an updated list of all unlicensed addresses and relocation sites of all residents. · Licensee to also submit a roster of all residents at each unlicensed location, and information on whether the residents went · Licensee already applied for these Unlicense facilities: Myler st, Carradale st and La Salle st; and Halldale. An exit interview and a copy of this report were provided to Lori and Charesa Reyes/License.
