What the inspector wrote

On 09/08/25, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Lilybelle Calzado, Lead Staff, and explained the purpose of the visit. The facility is licensed to serve (6) non-ambulatory residents ages 60 and above of which (1) may be bed-ridden. Facility has an approved hospice waiver for (3) patients. Currently there are six (6) residents in the facility. The facility is a single-story home located in a residential neighborhood. It consists of six (6) bedrooms, six (6) bathrooms, a front den / sitting area, living room, dining room, attached garage, and an outside shaded area. LPA Gonzalez and Lilybelle Calzado toured the physical plant. There were no bodies of water or obstructions on the premises. Exits/Walkways around the facility were free of debris and hazards. All resident bedrooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. Bathrooms were found to be operational and within Title 22 regulations. Smoke and carbon monoxide combo were tested and are working properly. The water temperature properly measured between 105 F-120 F. LPA observed the facility to be sanitary and appropriately furnished at the time of the visit. Storage areas for personal hygiene, medication, sharp objects and cleaning supplies were stored and inaccessible to residents. The kitchen was inspected and there is sufficient perishable and non-perishable food available maintained properly. Fire extinguishers were charged and were operable. Continued on LIC809-C LPA conducted a review of (6) resident service files, and (3) staff personnel files and (3) Medication Administration Record (MAR). LPA observed all records were maintained in order. A stocked First AID kit with manual was available. The most recent Fire/Emergency Drill was conducted on April 12, 2025. The facility's annual fees are current. During the visit, LPA observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents. There are sanitizing stations in common areas and restrooms. All mandated inspection control posters were posted. During this inspection, LPA did not observe any deficiencies, therefore no citations were issued at this time. An exit interview was conducted, and a copy of the report was given to Lilibelle Calzado.
