Inspector’s narrative

What the inspector wrote

On 09/17/2025, at approximately 11:00AM, Licensing Program Analyst (LPA) Jose Anguiano while touring the facility with Director of Resident Services, Broke Lamotte, observed that an emergency exit door leading to the alleyway, located near a resident’s room, was not in good repair. The door, which is equipped with a push-to-open mechanism, only opened approximately 30% of the way when tested. The door was visibly slanted and misaligned, restricting the opening to a narrow gap and gets stuck. This condition poses a potential hazard by obstructing safe and timely evacuation in the event of an emergency and does not meet the requirement for maintaining the facility in a safe and operable condition. According to California Code of Regulations, Title 22, Division 6, Chapter 1, the following deficiency was observed: Based on observations, the licensee did not comply with regulations by failing to maintain an emergency exit door in good repair. Plans of Correction were discussed and developed with the Director of Resident Services. A copy of this report, LIC 809-D, and Appeal Rights were provided.
