What the inspector wrote

On 08/20/2025, Licensing Program Analyst (LPA), Antonine Richard, conducted a case management visit to document deficiencies observed during an investigation related to Complaint Control Number:11-AS-20250812125512. LPA explained the purpose of the visit to staff member Girma Yodit. On 08/20/2025 at 10:00 am, LPA conducted a file review. LPA found that one of the facility staff members did not have background clearance and was not associated with the facility. Interviews conducted, observation, and records reviewed revealed that the facility was not in compliance with the California Code of Regulations Title 22. Deficiency cited based on interviews conducted, records reviewed, and observation in accordance with the California Code of Regulations, Title 22. A copy of the appeal rights and this report was left with the Staff Girma Yodit. On 08/21/2025, the license was cited with Title 22 Criminal Clearance Record Regulations 87355(e)(2). Based on interviews, observation, and record reviews, the licensee violated the California Code Regulations (CCR) of Title 22, Division 6, Chapter 8. Deficiency was issued. An exit interview was conducted with staff member, Girma Yodit. A copy of this report, appeal rights, and civil penalty was provided.
