On April 17, 2025, Licensing Program Analyst (LPA) Ernand Dabuet conducted a collateral visit to Happy Living Reliable Home LLC. Upon arrival, LPA Dabuet was received by the administrator, Gladys Cuela, who allowed entry to the facility. The primary purpose of this visit was to collect pertinent information regarding an ongoing complaint against Harmony Home Care, referenced as complaint number 11-AS-20250310121815. During the visit, LPA observed California Department of Social Services Community Care Licensing Investigator Edward Hector conducting an interview with Resident #1 (R1) and allowed for interview to proceed. After the interview with CDSS/CCL Investigator Hector, LPA Dabuet interviewed Resident #1 (R1), a former resident, to gather important insights. This interview is documented on LIC 812 and cross-referenced with records from Harmony Home Care #198601704. LPA Dabuet requested R1's current Physician Report (LIC 602A) and Harbor UCLA Medical Discharge records to validate information from the interview. An exit interview with Gladys Cuela and a hard copy of the report was provided.
