On 11/25/2025, Licensing Program Analyst (LPA) Jose Anguiano conducted an annual required visit to the facility. LPA met with Administrator Stephanie Slusher. The facility is licensed to serve residents aged 60 and over, with approval for four (4) ambulatory residents and a hospice waiver for up to four (4) residents. The facility is a single-story home located in a residential neighborhood. It includes (3) bedrooms, (1) bathroom, a living room, dining area, kitchen, outdoor seating area, and a shed. LPA conducted a tour of the physical plant. No bodies of water were observed on the premises. All rooms were inspected and found to be in good condition. Beds and bedding were in place and in good repair, with adequate lighting and storage for personal belongings. Clean linens, comforters, and towels were available. Bathroom water temperature measured 112F, and the facility maintained a comfortable indoor temperature of 76°F. The facility appeared clean, organized, and appropriately furnished. Storage areas for personal hygiene items and sharps were secured and inaccessible to residents. The kitchen was stocked with sufficient perishable and non-perishable food items. The fire extinguisher was fully charged. LPA observed the facility’s infection control practices, including visitor/staff/client screening protocols and accessible sanitizing stations in common areas and restrooms. The facility-maintained infection control signage at the facility. The First Aid Kit was fully stocked. A working landline phone was available. Smoke and carbon monoxide detectors were tested and operational in all bedrooms and common areas. Infection control & Emergency disaster plan was reviewed and maintained at the facility. Administrator certificate are valid and the facility is current on all Community Care Licensing Division (CCLD) annual fees. No deficiencies were cited during today’s visit. An exit interview was conducted with the Administrator, and a copy of this report was provided.
