Inspector’s narrative

What the inspector wrote

On 07/10/2025 Licensing Program Analyst (LPA) Jose Anguiano conducted a case management visit to the facility for purpose of a pre-licensing evaluation and to clarify items observed during the visit conducted on 03/06/2025 and to verify if corrections were made based on items observed. LPA met with Licensee Princey Hartman, and the purpose of the visit was explained. Per observation the following corrections from the previous pre-licensing visit were observed to be completed during today’s inspection and are no longer a concern: 1. Fireplaces are inaccessible to residents. 2. Lamps are available for residents in bedrooms. 3. Nightlight in the hallway with sensor has been installed. 4. Licensing Complaint Poster (20X26 inches) and Long-Term Care Ombudsman contact information has been posted on wall. 5. Grass in the backyard has been trimmed to avoid tripping hazards. 6. The power outlet in bathroom #1 has a cover plate. An exit interview was conducted, and a copy of this report was provided to Princey Hartman. Accordingly, LPA will submit a copy of this facility evaluation report to the Central Applications Unit (CAU) for review. If the applicant has questions regarding the status of the application, they have been instructed to communicate with the CAU Analyst assigned to the applicant.
