What the inspector wrote

On 08/22/2025, Licensing Program Analyst (LPA), Wendy Gibbs, conducted a Case Management to the facility listed above. The visit is a Case Management due to the facility appearing on the annual list when not requiring an annual. LPA Gibbs spoke with LPM Coronel who said to indicate the type of visit as a Case Management. LPA met with Administrator, Kelechi Eronobi, and the purpose of today’s visit was explained. LPA was granted entry into the facility. The facility is licensed to serve six (6) non-ambulatory residents over the age of 60, with an approved hospice waiver for six (6). There are currently three (3) residents residing in the facility. Physical Plant/Structure The facility is a single-story structure located in a residential neighborhood. It consists of 3 resident bedrooms, 3 1/2 bathrooms, living/common room/ dining room (open concept spacing), kitchen and attached garage. The garage has a washer, dryer, and is used for storage. All walkways around the facility were observed clean, clear and free of debris, hazards and obstructions. There were no bodies of water observed on the premises. Bedrooms LPA inspected the resident bedrooms and observed they have the required furniture including beds, dressers, night stands, chairs, and ample storage space for resident’s personal belongings. LPA observed the beds have the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillows. LPA observed an adequate supply of linens, in good repair. Bedrooms were observed with ample lighting. Bathrooms LPA observed the bathrooms meet Title 22 Regulations. The showers have non-skid mats and secured safety handrails. All bathrooms were observed operational. LPA observed an adequate supply of hygiene products available for resident use. LPA observed an adequate supply of towels, hand towels, and wash cloths in good repair. Kitchen The kitchen was observed clean and sanitary. All appliances were observed operational. LPA observed a 2-day supply of perishable foods and a 7-day supply of non-perishable foods. LPA observed an ample supply of cookware, dishware, and cutlery. LPA observed sharps secured in a locked drawer and are inaccessible to residents. LPA observed an ample supply of cleaning supplies secured in a locked cabinet under the kitchen sink and are inaccessible to residents. Common Areas LPA observed the facility appropriately furnished during the time of visit. The living room has a sectional couch and four (4) recliners available for resident use. LPA observed games, activities, and crafts on shelves in the living room. LPA observed the dining area has a large table and chairs to accommodate all residents. LPA observed all walkways and hallways in the facility are clean, clear, and free of hazards and obstructions. All rooms and hallways were observed with ample lighting. Safety LPA observed the smoke detectors and carbon monoxide detectors are operational. The facility has a working land line telephone. LPA observed 2 fully charged fire extinguishers last serviced on 12/05/2024. The First Aid kit had all the required items and a current manual. LPA observed all exits are marked with an EXIT sign. LPA observed all required postings posted in the facility. Medications LPA observed Centrally Stored Medications secured in a locked cart and are inaccessible to residents. LPA observed medications in their original packaging. Infection Control LPA observed in the entrance is a sanitizing station that consists of a visitor sign in log, thermometer, and hand sanitizer. LPA observed a 30- supply of Personal Protective Equipment (PPE) stored in the garage. LPA observed required infection control signs posted throughout the facility. During today’s visit LPA did not observe or cite any deficiencies. An exit interview was conducted with Administrator Kelechi Eronobi, and a copy of this report was provided.
