This requirement was not met as evidenced by: Based on interview and record reviews, the licensee failed to ensure that staff recognize early signs of the need for professional help, R1 continued to experience multiple falls that resulted to a left hip fracture on 12/05/2021, which poses an immediate health and safety risk to persons in care.
