What the inspector wrote

Licensing Program Analyst (LPA) Elizabeth Irra conducted the required annual inspection. LPA met with Jennifer Miano and discussed the purpose of today’s visit. Leah Avila (Administrator) arrived at approximately 8:20 A.M.. This is approved for (6) ambulatory residents. This home consists of (3) bedrooms, (2) bathrooms (1 of which is in between room #2 and room #3), (1) staff bedroom, kitchen, dining room, living room, laundry room and a detached garage. Frank D. Lanterman Regional Center provides case management services for all clients residing at this home. LPA utilized the Compliance and Regulatory (CARE) tools for the visit today and observed the following: Infection Control: Facility has an infection Control plan in place. Operational Requirements: Staff are adhering to operational requirements. Physical Plant & Environment Safety: The smoke and carbon monoxide detectors were tested and all are operable. The fire extinguishers were observed (kitchen and living room) and appeared to be full (last service date of 04/25/25). Hot water temperature measured at 118.8 in the hallway bathroom and 117.3* in the bathroom between bedroom #2 and bedroom #3. Knives, cleaning solutions, and disinfectants are locked and inaccessible to clients. Passageways were free of obstruction. Refer to LIC 809C for the continuation of this report. Staffing : There is sufficient staffing at the facility. Staff employed are over the age of (18) and are fingerprint cleared and associated to the facility. Personnel Records-Training : LPA reviewed staff files Staff #1 (S-1) through Staff #5 (S-5). Staff have current First Aid/CPR certification. Staff have their Health Screening and Tuberculosis results on file. Client Rights-Information: Resident rights observed in resident files. Client Records-Incident Reports: LPA reviewed Resident files for Resident #1 (R-1) through Resident #5 (R-5). The following observed: Admission Agreement, Physician's Report (including T.B and Ambulatory Status), Weight Record, Functional Capabilities Assessment, Consent For Medical Treatment, House Rules, Individual Program Plan, and Resident Rights. Food Service: There are sufficient food supplies of 2-day perishable and (1) week of non-perishable items. The food is properly stored in the refrigerator. Cleaning supplies are kept away from the food preparation areas. The kitchen is kept clean and free from rodents and other vermin. Plates, cups and utensils are kept cleaned and stored properly. Health Related Services: The medications are centrally stored and locked inside the kitchen cabinet. Medications are administered as prescribed by the Physician. Incidental Medical Services: There are (0) residents with restricted health condition plans in place, (0) residents with prohibited conditions and (0) residents with postural supports. Disaster Preparedness: The facility has an Emergency Disaster Plan in place. Exit interview conducted. A copy of the report and appeal rights were provided to Leah Avila.
