What the inspector wrote

On 01/09/2026, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced required – annual inspection and met with Administrator Teresa Guanlao . Facility is licensed to serve six (6) non-ambulatory residents over 60 years old. The facility is approved for six (6) hospice residents. The facility currently has two residents receiving hospice care. Annual Fees are current. The facility is a single-story structure located in a residential neighborhood. It consists of the following: four bedrooms of which two are private and two are shared, two bathrooms, a living area, dining area, kitchen, and outside covered patio area. The Administrator accompanied LPA inside and outside the facility during this inspection. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. Common areas were clean and clear of hazards, doorways were free of obstructions. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Continue to LIC809-C. There are no security bars or weapons on the premises. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secured, shower was free of mold/mildew, a non-skid mat was in place, and hot water temperature properly measured at 110-degree Fahrenheit. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. LPA toured the kitchen area and observed a two-day supply of perishable and a seven-day supply of non-perishable food. Knives and toxins were kept in locked storage cabinet. First Aid kit was available. One fire extinguisher, last serviced September 16, 2025 was observed in the living room. Staff tested the carbon monoxide detector and smoke detectors in the house. Devices were functional. Five staff records were reviewed, five out of five staff records had current first aid certificates and required criminal record clearances or criminal record exemptions. Five resident records were reviewed and, five out of five resident records had medical assessments and pre-appraisal or reappraisals. Two residents’ medication was reviewed. Deficiencies are being cited according to California Code of Regulations and Health and Safety Code, see LIC809-D. Record review of R1’s medical assessment revealed R1 is bedridden. Review of Fire Inspection Report revealed the facility is licensed for six non-ambulatory and zero bedridden. An immediate $500 civil penalty is issued, see LIC421IM. Record review of Staff #3 - Staff #5 (S3 – S5) did not reflect 20 hours of annual training. S3 and S4 had three to four hours of in-service training. An exit interview was conducted, plans of correction developed, and a copy of this report with appeal rights was discussed and left with Administrator Teresa Guanla .
