What the inspector wrote

Licensing Program Analyst (LPA) Daniel Konishi conducted an unannounced Required-1 year visit and met with Direct Support Professional, Leticia Aguilar Echeverria and LPA explained the purpose of the visit. Administrator Hector Vargas and House Manager, Wendy Vasquez, and Staff, Patricia Wong, arrived shortly after and LPA explained the purpose of the visit. The facility is licensed to care for (6) non ambulatory residents age 60 and over. The initial annual visit was conducted on 09/23/2025. During the initial visit the following nine (9) Compliance and Regulatory Enforcement (CARE) tool domains were observed and reviewed: Infection Control, Operational Requirement, Physical Plant Environmental Safety, Resident Right-Information, Planned Activities, Food Services, Incidental Medical and Dental, Disaster Preparedness, and Resident with Special Health Needs. During today’s annual visit, the following three (3) Compliance and Regulatory Enforcement (CARE) tool domains were observed and reviewed: Staffing, Personnel Records-Training, and Resident Records-Personnel Reports. Staffing: A total of nine (9) staff members including the Administrator provide care and supervision to the client. Staff employed are over the age of 18 and have criminal background clearance, fingerprint cleared, have training and are associated to the facility. [Continued in LIC809-C] Personnel Records/Staff Training: Staff files are maintained at the facility. LPA reviewed (4) staff files which includes: Personnel Record, Health Screening, TB Clearance, fingerprint background clearance, Employee Rights, valid 1st Aid/CPR training, and ongoing staff training. Administrator certificate expires on 06/23/2026. Resident Records-Incident Reports: LPA reviewed five (5) resident files which include: Face Sheet, Admission Agreement, Physician's Report (including TB and Ambulatory Status), Appraisal and Needs Services plan, Pre-Admission Appraisal (Physical/Social Assessment), and Resident Personal Rights. Per California Code of Regulations, Title 22, and California Health and Safety Code, no deficiencies observed during the visit. Exit interview and a copy of this report was provided to the Administrator, Hector Vargas.
