What the inspector wrote

On 5/04/26, Licensing Program Analyst (LPA) Mario Leon conducted an unannounced annual required visit using the CAREs Inspection Tool. California Department of Social Services (CDSS) met with Nino Echevarra - House Manager . CDSS explained the purpose of today’s visit. The facility is licensed to serve (6) residents ages 60 and above, of which (6) may be non-ambulatory and (1) Bedridden in bedroom #3. Approved hospice waiver for (6). Currently the facility has (4) residents. The facility is a single-story structure located in a residential neighborhood. It consists of (5) bedrooms, (2 -1/2) full bathrooms, back yard with shaded seating, front yard, and a laundry room in the attached garage. CDSS and the licensee toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (5) bedrooms and (2) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational and showers were without mold or mildew. Smoke and carbon monoxide detectors were in operable condition. The water temperature properly measured between 105F°. and 120F° (degrees Fahrenheit), and the room temperature was measured at 74.1°F. During the visit, CDSS observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. Report continues, please see LIC809-C. The kitchen was inspected, and there was sufficient 2-days' perishable and 7-days' non-perishable food available, all of which was adequately maintained. All fire extinguishers were charged and operable, last inspected 09/12/25. A review of four (4) residents' service files and two (2) staff personnel files was maintained in order. CDSS found no discrepancies. During today's inspection, there have been zero (0) deficiencies cited. An exit interview was held with Nino Echevarra (S1) and a copy of this report has been provided.
