On 05/14/2025 , Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Wilma Iglesias LPA explained the purpose of the visit facility is licensed to serve a total of 6 adults ages 60 and above, of which 5 may be non-ambulatory residents. There is a hospice waiver for 2. A total of 6 residents are currently residing in this facility. The Annual Licensing Fees are current. Structure/physical plant: This home consists of 4 client bedrooms and 3 client bathrooms, living room, kitchen, dining area, activity room, laundry area and patio. There were 5 residents present during the inspection. Kitchen: LPA toured the kitchen area and observed supplies of nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days. Knives and toxins were kept in locked storage cabinet. Page 1 of 2 Bedrooms/Bathrooms: All 4 bedrooms were inspected. The mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked. Bathroom toilets and water faucets worked properly, grab bars were secure, and a non-skid mat was in place. Adequate lighting and toiletries accessible to residents. LPA tested hot water temperature, and it measured between 105 and 120 degrees Fahrenheit. This facility provides residents with hygiene products such as feminine napkins, soap, toilet paper, toothbrush, toothpaste, and comb. Safety: LPA inspected the First Aid kit and found it contained an ample supply of required items: scissors, tweezers, gauze, disinfectant wipes, band aids. LPA observed all exits to be clear and easily accessible. All toxins locked and inaccessible to residents in care. LPA observed all required documents posted throughout the facility. The facility has a working landline telephone. Due to time restraints, LPA to complete inspection on subsequent visit. There were no deficiencies cited during today’s visit. An exit interview was conducted with Wilma Iglesias and a copy of this report was provided. Page 2 of 2
