What the inspector wrote

The investigation revealed the following: Based on statements and interviews conducted with staff all residents are changed frequently and are check on every 2-3 hours. One (1) out of (3) resident interviews reveled they are not changed often. Based on interviews conducted there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there are not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED An exit interview was conducted with Gerardo Alfonso and copy of this report was provided.
