What the inspector wrote

Licensing Program Analyst (LPA) Erik Zaragoza conducted a case management visit in conjunction with a complaint that has the control # 28-AS-20251210115908. During the course of the investigation, a total of five (5) residents and one (1) staff member corroborated that a staff member has handled residents in a rough manner in the past. Therefore a resulting deficiency is being cited on the LIC809D page. A copy of this report along with the appeal rights were provided.
