Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced case management visit to this facility . During a complaint investigation conducted on 7/7/21, LPA discovered that Staff / Estephanie Karina Morales was employed and working at this facility prior to obtaining a California clearance or a criminal record exemption as required by the Department. Upon checking the staff file for Estephanie Karina Morales, LPA did not observe a criminal record clearance and after checking the CCL database, there was no record found for a Estephanie Karina Morales. The date of hire for Estephanie Karina Morales was on 3/9/21. The following deficiency was observed to be in violation of California code of Regulations, Title 22, Division 6 (refer to 809D) An exit interview was conducted and a copy of this report was provided to the Assistant Administrator, along with the Appeals Rights.
