Staff interviewed indicated they had been fingerprint cleared prior to working at the facility. 2 of the staff acknowledged S1 had worked at the facility in the past to cover a shift. Residents interviewed could not recall S1 working at the facility. Based on the Licensing roster, S1 has background clearance but not associated to the facility. Based on record review and interviews, the preponderance of evidence standard has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6 and Chapter 8), are being cited on the attached LIC 9099D. A deficiency is issued on the LIC9099D and a civil penalty is also issued. An exit interview was conducted. A copy of this report and appeal rights were provided to Staff.
